Department of Health and Human Services Integrated Care Model Steering Committee

DHHS is seeking a consumer for their Victorian Integrated Care Model (VICM) Steering Committee

Background and Purpose

The purpose of the VICM Steering Committee is to assist with strategic oversight and direction for the delivery of the VICM. Its aim is to improve the experiences of care and outcomes for patients with complex and chronic conditions through increased collaboration by health care providers at the local level.

The Consumer Role

Consumer participation provides the opportunity for real-time and relevant feedback to inform the development of the VICM and ensure it meets patients’ needs and preferences. We look to the consumer to offer insights into the experience of a patient and carer as well as a unique perspective to ensure implementation of the VICM improves the patient experience.

Previous experience on government working groups or steering committees in health service design would be valuable. Knowledge of the health system (across primary and acute care) is preferable. The consumer will raise emerging project issues with wider implication for patient safety and quality and ensure their advice and information supports patient-centred care.

Meetings

The next meeting is scheduled to take place on June 17, 2019 with additional meetings to follow every 3 months until 30 June, 2020. All meetings will take place at DHHS Head Office, 50 Lonsdale St, Melbourne. Teleconference facilities are also available.  

Remuneration

Sitting fees, travel  and parking expenses will be covered.
